ID cards display the policyholder’s name, but are used by all family members who are covered under the policy.

ID cards are sent in sets of two. You can obtain a digital copy of the ID card by signing into your Delta Dental Member Connection account. You can also call Delta Dental of Iowa to request additional cards. Contact Delta Dental of Iowa at 1-800-544-0718.
